Stranded in Space for 9 Months: Before-and-After Photos Reveal Toll on Astronauts’ Bodies

When astronauts Barry “Butch” Wilmore and Sunita “Suni” Williams blasted off to the International Space Station (ISS) in June 2024, they were prepared for a brief eight-day mission. Instead, they found themselves stranded in orbit for an astonishing nine months due to a cascade of unexpected complications. On Tuesday, March 18, 2025, their long-awaited return to Earth finally arrived — and the photos of their homecoming tell a powerful story of resilience, endurance, and the high cost of space exploration.

The two NASA veterans made their return aboard the SpaceX Dragon spacecraft, splashing down off the coast of Florida. But rather than stepping triumphantly onto solid ground, both Wilmore and Williams had to be carried out on stretchers. After 270 days in microgravity, their bodies could no longer immediately support the weight of Earth’s gravity — a stark reminder of how space travel reshapes the human body.

Before-and-after images of the pair reveal the physical toll of their extended stay in space. Once strong and robust, Wilmore and Williams now appear visibly gaunt, their frames thinner and their facial features more hollow. Experts note that weight loss in space is common due to muscle atrophy and shifts in body fluid distribution, but nine months is a long time — even for trained astronauts.

NASA’s Johnson Space Center in Houston is now hosting the two for an intensive period of medical testing and rehabilitation. Their journey back to full health will include physical therapy, cardiovascular monitoring, and numerous tests to assess the long-term impact of spaceflight on their bodies. But early signs are already raising concerns among some specialists.

One key condition under scrutiny is Spaceflight Associated Neuro-Ocular Syndrome (SANS) — a disorder triggered by prolonged exposure to microgravity, which can cause fluid to build up in the skull, placing pressure on the eyes and optic nerves. This can result in vision changes, swelling around the eyes, and other facial changes. While most astronauts eventually recover from SANS upon return to Earth, NASA has warned that some side effects can be permanent — particularly for those who have spent extensive time in orbit.

The absence of gravity also wreaks havoc on muscle and bone density. In space, muscles that support posture and movement on Earth essentially “go to sleep.” This muscle atrophy leads to conditions astronauts informally refer to as “chicken legs” — visibly thinner limbs due to loss of muscle mass. Their feet also soften, earning the nickname “baby feet,” since they’re no longer walking or putting pressure on the soles.

To add to the list of physical challenges, experts have highlighted Spaceflight Venous Thrombosis (SVT) — a condition caused by fluid shifting upward in the body, making the face appear swollen or “puffy.” Combine that with the rapid weight loss and muscle deterioration, and the change in appearance can be striking.

Despite these challenges, Wilmore and Williams remained mentally sharp and in good spirits throughout their ordeal. Their professionalism and resilience have drawn admiration from both NASA colleagues and the wider public. But their return also raises broader questions about the risks of long-duration space missions — particularly as space agencies look toward future missions to the Moon, Mars, and beyond.

The duo’s prolonged mission wasn’t part of the original plan. They were launched aboard Boeing’s Starliner spacecraft — part of a test flight meant to prove the vehicle’s readiness. However, various technical issues with the Starliner left the crew unable to return as scheduled. NASA and its commercial partners worked around the clock to resolve the issues, and the eventual decision was made to bring the astronauts home via a different spacecraft: SpaceX’s reliable Dragon capsule.

While Wilmore and Williams are now safely back on Earth, the long-term consequences of their extended space stay are still being evaluated. NASA says they will continue to monitor both astronauts’ health closely and gather data that could be crucial in understanding how the human body copes with extended spaceflight.

For now, the striking before-and-after images of Wilmore and Williams serve as a visual testament to the physical extremes endured by those who venture beyond our atmosphere. They are reminders not just of the cost of space travel, but of the courage and sacrifice required to explore the final frontier.
